PARENTAL WAIVER FORM
It includes participant and event information, voluntary participation, ordinary event risks, emergency contact and accessibility/accommodation information, personal property, release language, photography/video permissions, special parental/legal guardian consent for participants under 18, signatures, and AYCEO contact information.
NOMINEE APPLICATION FORM
It includes nominee and business information, nomination source, business stage, verification evidence, leadership and impact questions, first-dollar/customer story, 300–600 word impact statement, optional video, public-profile permissions, guardian requirements for applicants under 18, certification/signatures, and a submission checklist.
SPONSORSHIP FORM
The form includes sponsor contact information, financial and in-kind sponsorship options, partnership interests, sponsorship goals, organization/logo permissions, AYCEO's ethical sponsorship commitments, authorized signature, and an internal follow-up area.
MEDIA KIT
The kit includes the program overview, mission and vision, AYCEO-at-a-glance facts, media story angles, recognition and verification, youth protection/ethics/inclusion, sponsors and community partnerships, press contact information, a media boilerplate, website QR code, and branded imagery.
